Please fill out this form for each TT you offer AT LEAST TWO WEEKS before graduation to allow us adequate time to print and send diplomas in time for your graduation.
Please submit this form, along with a completed list of graduate names (as they’d like for them to appear on the diploma), email, phone numbers, address. Send to [email protected]
Upon receipt of this form, IYTA will:
• Calculate the tithe amount due
• Send an invoice via email to the Center/Institute Director
• Send graduation certificates via postal mail to the designated contact person. (Express mail options are available; the additional cost for express mailing will be added to the amount due.)
Pay the amount due upon receipt via check or PayPal (under “Pay for Goods and Services”, enter “[email protected]” and the tithe amount due”). Alternatively, you may call +1 (434) 969-3121 x177 to pay by credit card.
